Revisions to 14 protocols mandate the Breast Phyllodes Tumor protocol for accreditation and update ER, PgR, and HER2 scoring criteria.
The College of American Pathologists (CAP) has released quarterly updates to 14 of its Cancer Protocols, introducing changes that alter breast cancer biomarker reporting and molecular testing for gastrointestinal stromal tumors (GISTs). The revisions are designed to align clinical pathology reporting standards with advances in precision oncology and evolving World Health Organization (WHO) classifications.
Breast Cancer Protocol Updates and Accreditation Mandates
Among the primary modifications, the Breast Phyllodes Tumor protocol is now required for laboratory accreditation when reporting malignant phyllodes tumors, CAP stated in a release. The revised protocol incorporates WHO 6th Edition updates, changes to reporting elements, and updated criteria for cancer staging and surgical margin reporting.
The Q3 release also updates the Breast Biomarker Protocol, adjusting the scoring and reporting criteria for estrogen receptor, progesterone receptor, and HER2, according to the organization. These modifications are intended to foster consistent reporting of critical biomarkers used to guide breast cancer treatment selection and clinical care decisions.
Expanded Molecular Testing for GISTs Across Anatomic Sites
In addition to the breast cancer protocol changes, CAP expanded molecular testing and reporting options within its GIST protocols, according to the release. The updates provide new reporting selections for NTRK fusion gene analysis alongside enhancements to KIT and PDGFRA biomarker testing and reporting.
Furthermore, the updated protocols now support pathology reporting for GIST cases across all anatomic sites, the organization reported.
Standardizing Precision Oncology Reporting
CAP updates its Cancer Protocols quarterly to incorporate guidance from standard-setting bodies, including the WHO and the American Joint Committee on Cancer, according to the release. These evidence-based reporting tools are designed to assist pathologists and clinical laboratories in delivering standardized cancer reports that support clinical staging, diagnosis, and patient management.
